Aneurin Bevan University Health Board is seeking experienced Registered Nurses (RGN, RMN, or RNLD) for the role of Complex Care Nurse Assessor in Pontypool. The primary responsibilities include conducting in-depth assessments of individuals with complex health care needs within the community, specifically in nursing homes and hospitals across Gwent. The role aims to determine eligibility for Continuing NHS Healthcare and related funding while facilitating a Multi-disciplinary Team assessment process and monitoring care packages.

Ideal candidates should have recent NHS or care home experience, demonstrate reliability, and possess a flexible approach to work.
