The Leeds and York Partnership NHS Foundation Trust is seeking a highly motivated Band 5 Nurse for the Memory Nurse role in the Memory Assessment Service, covering South Leeds. The position involves assessing, diagnosing, and providing post-diagnostic support for individuals with dementia. The nurse will collaborate with a multidisciplinary team, follow intervention plans, and engage in professional development, clinical supervision, and service improvement initiatives. Applicants should demonstrate a passion for working with older people, strong clinical judgment, and communication skills. A full UK driving license and access to a car are essential, as the role requires travel.
