The Lead Nurse in the Dementia Rapid Response Team at Derbyshire Healthcare NHS Foundation Trust in Bakewell is responsible for delivering innovative, person-centred care to older adults experiencing dementia-related mental health crises. The role involves working collaboratively within a multi-disciplinary team, managing complex cases, and implementing non-pharmacological interventions to maintain patients’ independence and dignity while aiming to prevent hospital admissions and facilitate early discharges. The service operates year-round, and the Lead Nurse is expected to take on clinical responsibilities such as triaging referrals, conducting assessments, and formulating safety and care plans tailored to individual needs.

Candidates should be experienced mental health nurses with a passion for supporting individuals living with dementia. The position requires strong decision-making skills, leadership during shifts, and effective communication with various professionals and agencies.
