The role of the Community Palliative and End of Life Care Lead at Kingston and Richmond NHS Foundation Trust in Teddington focuses on overseeing and enhancing the end-of-life care program within the Richmond community. The lead will be responsible for developing and implementing the Trust’s end-of-life care strategy in alignment with national guidelines while addressing local needs. Key responsibilities include collaborating with clinical leaders to ensure high-quality, coordinated care, monitoring performance through key performance indicators (KPIs), and providing governance by conducting clinical audits and maintaining an up-to-date risk register.

In addition, the role involves facilitating staff training and professional development, acting as the clinical leader for end-of-life care initiatives, and ensuring best practices are integrated into care planning. The lead will also engage with partner organisations to foster a seamless care experience for patients and coordinate performance reporting to support informed decision-making for continuous service improvement.
