MENTAL HEALTH NHS SOCIAL WORK
The Sussex Partnership NHS Foundation Trust is seeking a Mental Health Liaison Practitioner for their Crawley Mental Health Liaison Practitioner Service, offering both permanent full-time and part-time positions. Candidates must be registered mental health nurses, social workers, or occupational therapists. The role involves working collaboratively with primary care teams, including GPs and IAPT colleagues, to assess and manage patients with functional mental illnesses. Responsibilities include providing clinical support and consultation, facilitating patient care pathways, conducting assessments for referrals to secondary care, and ensuring communication between primary and secondary services.

Additionally, the position provides mentorship, supervision, and opportunities for professional development, with work hours typically from 9 am to 5 pm, Monday to Friday. The Trust offers a £4,000 incentive for eligible practitioners and a relocation package of up to £8,000, subject to terms. The role emphasises a patient-centered approach, evidence-based practice, and compliance with relevant guidelines and Trust policies, while also fostering collaboration and training within primary care teams.
