Wootton Vale and Shortstown Surgery is a forward thinking and innovative practice looking to recruit a health care coordinator to join our dynamic team on a full or part-time basis, and although this is a permanent position, we will consider flexible contract options. Part-time hours will need to be a minimum of 28 per week.
We are looking for an organised, self-motivated, and care-driven individual, to join our dedicated clinical liaison team. We are seeking an individual who can demonstrate a talent for helping our patients to achieve the best clinical outcome and who is looking to build a career in the healthcare sector. If you are organised, flexible and resourceful, and can work under pressure in a busy environment then we'd love to hear from you.
- Manage referrals via the appropriate system for clinicians.
- Follow up referrals if required.
- Coordinate the referral process and ensure all referrals are received.
- Coordinate birthday QOF reviews for all long-term conditions.
- Generate ICE forms for all blood tests.
- Generate invites and co-ordinate booking of appointments for these reviews.
- Arrange home visits for housebound patients.
- Maintain obesity register.
- Report regularly to immunisation lead if these targets are lowering.
- Monitor the non-attenders and follow all DNA pathways as per protocol.
- Order flu vaccines.
Some flexibility is required as you will sometimes need to work additional or alternative hours to cover for colleagues.
