Sussex Community NHS Foundation Trust is inviting dynamic and passionate Band 5 Occupational Therapists to embrace an exciting role within our innovative Home First service, based in Horsham. This unique opportunity allows you to play a vital role in facilitating patient recovery post-hospital discharge directly in their own homes. You will be integral in delivering community-based assessments and bespoke treatment programmes, empowering patients to achieve independence in their daily lives. Our multidisciplinary teams are united in providing exemplary care, offering a continuous service every day of the year from 8am to 8pm, ensuring timely intervention and reducing the risk of re-admission.
We are seeking professional, proactive therapists who are driven to help others and have a keen interest in rehabilitation and reablement. You will manage a diverse caseload, applying evidence-based practices and clinical reasoning to formulate effective treatment plans. A full UK driving licence and access to a vehicle are vital due to the service area's geographical spread.
