Band 6: £37,338 to £44,962 a year per annum, pro rata  PERMANENT  GOOD SALARY   NO SPONSORSHIP 

This is an exciting opportunity to join the Diabetes and Nutrition Services (DANS) in one of the largest community services in England. We are currently a team of 33 staff including Band 5, 6 & 7 Dietitians, Diabetes Specialist Nurses, and Administrators.

We are looking for an enthusiastic, motivated, and innovative Dietitian to join our team of multi professional staff.

You will have the opportunity to work across different settings including rehabilitation units, residential care, patient's own homes, and clinics.

As a Specialist Service we work closely with the community learning disabilities team, tissue viability, and respiratory specialist services, community integrated network teams, and primary, secondary, and social care. We also in-reach into the sub-acute stroke rehabilitation unit and community stroke rehab team which requires the management of enteral feeding post stroke and transition of patients back home.

We are looking for someone who has a passion to work in the community to improve health outcomes across BNSSG. The role is suitable for a band 5 looking for the next step in their career, or a band 6 looking for a fresh challenge. We will consider full time and part-time working.

DANS is committed to CPD. We have good clinical supervision models and in-house CPD in place to support you, including regular team MDT sessions.

The successful candidate will have experience in a clinical area(s)within the DANS pathways, and demonstrate good leadership and team working skills with a practical, forward-thinking approach to service delivery and improvement. They will also be an excellent communicator with the ability to work in a holistic manner and build relationships with a variety of stakeholders.

This role provides an opportunity to work with and supervise junior colleagues. Due to the service having a number of different pathways there is opportunity to specialise in different areas within the service. Wherever possible, workplans will be tailored to the successful candidate's skills, interests and prior experience.

You will be required to:

  • Assess and support a community caseload of patients with a range of complex needs and a variety of clinical conditions such as neurological diseases, Irritable Bowel Syndrome, Diabetes, and adults with a learning disability.
  • Deliver patient and/or health care professional training sessions on nutrition-related topics including structured patient education across BNSSG for type 1 and type 2 diabetes alongside the services specialist nurses.
  • Conduct specialist, evidence-base dietetic consultations which provide holistic, patient-centred care, supporting self-management strategies.
  • Participate in audit, research activities, pathway and policy development, and student training.
