To provide specialist assessment, diagnosis and intervention for adults with acquired speech, language, communication and swallowing difficulties.
To work as an autonomous practitioner within the standards of professional practice stipulated by the Royal College of Speech and Language Therapists (RCSLT)
To be proficient in managing a complex adult caseload of a variety of long-term conditions including dementia.
To work as a member of the multidisciplinary team, initiating, participating and supporting line manager in team development and service improvement, ensuring that systems and processes are in place to deliver an efficient and effective service.
To act as a specialist in an adult SLT community service, acting as a resource and support for other therapists and professionals working with adults and older people with acquired speech, language, communication or swallowing disorders.
To be responsible for the delivery of the Speech and Language Therapy service provided by Maximising Independence Team, providing assessment, implementing specialist SLT treatment and programmes of care, writing reports, reflecting specialist knowledge.
To provide specialist assessment, diagnosis and intervention for clients with complex acquired speech, language and swallowing difficulties, who require a domiciliary service. These adults will present with a wide range of cognitive, sensory, physical, behavioural, social, and communication difficulties.
To conduct interpretive and functional assessment using specialist clinical/technical skills, demonstrating highly developed auditory, perceptual and physical skills in the assessment, diagnosis and treatment of clients. Furthermore, to use investigative skills and specialist knowledge of standardised assessments and other assessment tools, together with advanced analytical clinical reasoning skills in order to form a baseline measure or differential diagnosis from which to plan intervention.
To be professionally and legally accountable for all aspects of your own work, including the management of patients within your care, complying with Health Professions Council Standards of Proficiency, Conduct, Performance and Ethics and abiding by the Royal College of Speech and Language Therapy rules of professional conduct.
To participate in continuing professional development and other activities to maintain evidence based best practice.
To share responsibility with colleagues for the day to day provision of the service and for the supervision of more junior level staff and students as required.
To maintain up-to-date records of your intervention according to professional and Trust standards, including goal setting, risk assessment, reporting writing and statistics.
To maintain a professional portfolio for CPD, recording learning outcomes through participation in internal and external development opportunities.
To promote and maintain professional standards across the team through supervision and audit.
