Clinical Nurse Lead - Prevention of Falls

In this post, the post holder will act as a clinical expert and be instrumental for ensuring that an integrated falls strategy is continued at Medway NHS Foundation Trust which will meet National and Local drivers in reducing falls and falls with harm within the hospital and local community.

About usLead the Falls Service across specified areas of responsibility to ensure a unified approach to care, consistent standards of care and patient access.
To lead on the development of appropriate policies and standards relating to services auditing results and responses.
To review local services for falls and fractures to identify gaps and to establish a baseline for service delivery, including the identification of multi-agency resources currently supporting Falls and fractures
Influence the care provided by other Health and Social care professionals through provision of guidance and involvement in formal teaching
Demonstrates a highly developed specialist knowledge within own discipline underpinned by current evidence-based practice acting as a reference for colleagues and other agencies.
To actively engage with clinicians and all key stakeholders and to foster good working relationships to ensure effective partnership approach to achieving multi agency outcomes
To coordinate and manage the ambulatory falls care pathway, triaging patients onto an appropriate clinic or activity programme.
The post holder is responsible for supporting the senior sister charge nurse and multi-professional teams In the day to day delivery of care, ensuring optimal high quality fall prevention and falls management is delivered and continuously monitored.
