Northumbria Healthcare NHS Foundation Trust is seeking a permanent Band 5 Children's Community Nurse to join their Children's Community Nursing team in Ashington.
The role focuses on delivering skilled, evidence-based nursing care to children and young people aged 0-18 with various health needs, in their homes and community settings across Northumberland and North Tyneside.
Responsibilities include assessing, planning, implementing, and evaluating care packages for children with acute and chronic illnesses, disabilities, or palliative needs.
The nurse will collaborate with families and other professionals from health, social services, and education to provide tailored care, also supporting transitions from children's to adult services.
The role emphasizes compassionate leadership, team collaboration, and responsive care delivery, including work in special schools and paediatric clinics.
