The Lead Nurse in the Dementia Rapid Response Team at Derbyshire Healthcare NHS Foundation Trust is responsible for delivering innovative, person-centred care to older adults living with dementia. This role involves managing complex cases within a multidisciplinary team that includes various healthcare professionals, and the aim is to maintain patients' independence and dignity while preventing hospital admissions whenever possible. The Lead Nurse will triage referrals, conduct initial patient assessments, formulate care plans focusing on non-pharmacological interventions, and ensure effective documentation.

The service operates 365 days a year, offering flexible support to individuals in their homes or care facilities, and emphasises collaboration with families and caregivers. Candidates for this position should be experienced mental health nurses with a commitment to improving the quality of life for those with dementia, capable of making complex decisions, leading shifts, and actively participating in ongoing professional development through training and supervision.
