Job description
We have an exciting opportunity to recruit a Band 5/6Respiratory Nurse to join the Community Respiratory Team within the Weymouth & Portland locality. This will be a collaborative role working closely with GP Practices, the Local Trust and Secondary Care.
To assist in the delivery of community service provision for patients with acute/chronic respiratory diseases. The post holder will support the work to develop seamless care for this patient group between primary care, integrated community care and secondary care.
Work with the lead nurse and other team members to ensure high standards of nursing care are maintained in the management of patients with respiratory disease.
Provide specialist advice as requested/required across the locality, within own scope of practice.

To provide a high standard of evidence-based care for respiratory patients, carers and other health professionals, contributing to the effective management of both acute and chronic phases of respiratory problems in the community.
