Band 3: £24,071 to £25,674 a year pro rata  PERMANENT  GOOD SALARY 

The successful candidate will support nursing staff and deliver independently a high standard of nursing care for patients in their own homes.

This role as a band 3 Health care assistant offers lifestyle flexibility as part of the evening team working 18:30-23:30 There is 1 post of 15 hours available working 3 shifts a week each.

Although the team is a countywide team with a central base in Droitwich, there are several satellite bases across the county. This vacancy is for the South of the County being based Droitwich Medical Centre.

The Team visits patients in their own home and support them in remaining at home and preventing admission to hospital, and also supporting early discharge from hospital.

The post is all worked as unsocial hours hence picking up pay enhancements for all hours worked which makes this post more attractive.

As a HCA on the evening team you will be allocated via our electronic system onto a handheld tablet, patients to visit in their own homes, primarily across the South of the county. These would normally be within a geographic area, but some flexibility is expected to cover patients countywide. The role of the HCA is varied, in supporting patients in their own home with personal care, mobility, and simple wound care and catheter care. All appropriate training will be provided as part of the induction process. You will work independently but also as part of a team, with some visits requiring 2 members of staff. You will be supported during the shift by a senior nurse on triage.

You will be joining the Evening and Night team element of one of our dynamic and supportive Neighbourhood Teams to provide a 24-hour admission avoidance service to the population of Worcestershire which encompasses both urban and rural areas. The Service is based at Droitwich, with further satellite bases across the County and flexibility to start from home.

The successful candidate will support nursing staff and deliver independently a high standard of nursing care for patients in their own homes. Ensuring that care provided maintains the individuals privacy and dignity, values diversity and respects cultural and individual rights and choices.

We are looking for candidates with good communication skills, a flexible approach and enthusiasm to work within the team. You should be a committed, highly motivated and forward-thinking individual who is a good team player.

The successful candidate will also assist the team to develop and achieve team and organizational objectives. They will also participate in staff meetings and training & development activities as necessary to ensure their skills and knowledge are up to date.

Due to the nature of the post the applicant must be able to travel independently and have access to a suitable vehicle for business purposes. Whilst a base will be identified, staff can be asked to work anywhere within the Business Unit or Service if necessary.
