Band 6: £37,338 to £44,962 a year per annum (pro rata)  PERMANENT 

The Urgent Community Response (UCR) Sister will continuously assess the needs of the individuals with long term conditions, complex health needs and acute episodes of ill health requiring an urgent 2 hour response, and agree care plans, utilising the single assessment process.

They will work with the area professional leads and in partnership with GP practices and the local Primary Care Networks to further develop services which are of value to the commissioners and meet the local needs of the population.

The post holder will work autonomously as part of the Community UCR to provide a comprehensive assessment and patient centred care package, utilising support services and liaising with other agencies to ensure the needs of this client group are met.

The UCR Sister will have physical assessment and disease management skills and will evidence management of acute/chronic clinical conditions, multiple co-morbidities and poly-pharmacy, within the home environment, ensuring an optimum standard of patient centred care.

Successful candidate must have access to a vehicle for work purposes to meet the requirements for the role.

  • To be responsible for monitoring and supporting patients under the care of UCR team (previously known as Rapid Response/Fast Response caseload) to provide appropriate management, treatment and advice to patients and provide clinical support for the associated nursing and therapy staff.
  • To have the physical assessment and disease management skills to able to deliver an effective clinical assessment providing an urgent crisis response (UCR) to all patients in the Redcar and Cleveland, Middlesbrough and Hambleton and Richmondshire localities within 2 hours of initial referral.
  • Working autonomously, the post holder will have continuing responsibility for pro-actively monitoring and supporting patients within the UCR service.

The post holder will be expected to work flexibly across 7 days a week, ensuring the 24 hour service is covered, to meet the changing needs of the service, which may include working across the Middlesbrough, Redcar and Cleveland and Hambleton and Richmondshire area.

This role involves both day and night shifts.
