We are offering an exciting opportunity for a Band 6 Community Mental Health Practitioner to work within our Intensive Life Skills Pathway based in South Staffordshire (Lichfield and Tamworth, Burton and Uttoxeter, Cannock, and Stafford and Seisdon).
This service is expanding, and it is an exciting time to join the service as a highly skilled NMC Registered Nurse, HCPC Registered Occupational Therapist or Registered Social Worker.
The Intensive Life Skills Pathway is a multi-disciplinary Mental Health Team providing assessment and intervention for adults experiencing complex emotional difficulties as part of the Adult Mental Health Service.
The team works in a highly psychologically minded way and are enthusiastic about working with service-users who present with high levels of emotional distress and often have histories of complex trauma and/or adverse experiences.

A range of additional therapeutic models are utilised including attachment theory, behavioural theory, Compassion-Focussed Therapy, EMDR, Trauma-Focussed Cognitive Behavioural Therapy, Mentalisation-Based Treatment, and DBT informed interventions.

As a Mental Health Community Practitioner you will be expected to undertake holistic comprehensive assessments and formulation of complex clinical presentations, including risk assessments and risk management.
The role will involve:
Delivering group based and individual interventions in line with the pathway model and utilising Structured Clinical Management (SCM). There will be a requirement to undertake the role of named worker for a caseload of service users.

Responsible for care coordination of an identified caseload.
