An exciting opportunity for a specialist pharmacist in VTE Prevention and Thromboprophylaxis has arisen within our team at The WhittingtonHealth NHS Trust.

We are looking for a fixed-term pharmacist to cover for one year whilst our current post-holder goes on maternity leave.

Do you want a role that involves patient interaction alongside teaching, audit, service evaluation and qualityimprovement?The successful candidate will be responsible for developing and promoting best practice in thromboprophylaxis and VTE prevention at theWhittington Hospital. They will also be leading on audits and investigations into hospital-acquired venous thromboembolisms.

The successful post-holder will act as an expert clinical practitioner developing and promoting best practice in VTE prevention. They will provideadvice and training via Mandatory Trust training to medical and Allied Health care Professionals.The post involves working closely with Thrombosis lead and patient safety teams but will also be independent and autonomous with its own dailyworkload.

Main duties include:o Continued implementation of NICE guidance in reducing the risk of VTE across the Trusto Monitor electronic VTE risk assessments and deliver engagement and surveillance in high admission areas optimising risk assessmentfor all patients.o Patients who develop a hospital acquired VTE, the post holder will work within multidisciplinary teams to complete root cause analysisinvestigations, reporting back to stakeholders all outcomes.

MAIN DUTIES

Clinical / Operational responsibilities:1. To work with clinical teams in ensuring all in-patients receive an electronic VTE risk assessment (currently on Careflow but to alsoexplore other electronic options available including System C) and auditing the outcomes in accordance with NICE guidance andlocal commissioner requirements2. To improve and sustain compliance with recommendations from the Government Select Committee report / VTE Expert WorkingGroups for DoH in relation to VTE protocols /guidance.3. To work with data managers at Whittington to ensure that data being reported is clean and correct and therefore accurate for use inaudit4. To audit VTE risk assessment numbers on a monthly basis and feedback to Thrombosis Committee, Medicines Safety Committee andlocal commissioners where required5. To provide information and data in a timely manner for all freedom of information (FoI) requests6. To be responsible for the carrying out and management of root cause analysis (RCA) on all hospital acquired thromboses and to reportfeedback directly to the Thrombosis Committee and Medicines Safety Committee7. To alert all clinicians where VTE are confirmed through hospitalisation and incorrect / sub-therapeutic treatment has taken place,offering support and advice to clinical groups in these situations.8. To be a source of information and provide expertise in VTE risk management to acute staff in the prevention, diagnosis and treatmentof VTE9. To support the MDT in delivering organisational VTE prevention objectives and meeting national/locally agreed standards10. To work with the Thrombosis Committee and relevant clinical teams in creating robust care plans/guidelines for all patients admitted11. To develop and implement written information pertaining to risks of VTE and ensure this is available in all clinical areas for staff,patients and general public12. To support good communications and working relationships across the health sector divide. Ensuring both the hospital discharge teamand local primary care partners are supported in the continuation of care when patients are discharged from hospital with extendedthromboprophylaxis

Policy, Service Development and Professional responsibilities1. To follow both the Trust and national policy in line with VTE prevention2. To be motivated and enthusiastic toward the VTE prevention and thromboprophylaxis program instilling a positive attitude throughoutthe Trust.3. To be a key member of the Thrombosis Committee and participate in policy developments, delivering education needs and auditchange in clinical practice4. To be a key player in auditing of all aspects of VTE. This will include Trust performance (KPI and locally commissioned) and financialbenefits where required5. To develop and maintain links within national and regional clinical networks to enhance and optimise the delivery of VTEthromboprophylaxis and VTE management6. To work to implement changes VTE policies in line with agreed Trust and national policies for thromboprophylaxis.7. To participate in the education of medical, nursing / midwifery and pharmacy staff by organising and delivering training in relation toVTE8. To participate and attend specialist education forums / meetings. Disseminating all knowledge and information gained.9. To share and promote good practice10. To ensure VTE services provided are in line with local and national policies, guidelines and frameworks.

Personal Development1. All staff are required to be appraised by their line managers at least once a year at a personal development review meeting whereprogress made over the last year is discussed and agreed. Focus on the following years departmental and personal objectives will beidentified, discussed and agreed. Where necessary, help and support will be provided and development opportunities agreed in linewith service provision and knowledge and skills competency framework.2. To take responsibility for undertaking induction and mandatory training in line with the trust guidelines.3. To take responsibility in keeping up to date within specialist field of practice and to be proactive in identifying training and educationprograms to support individual needs

To carry out any other reasonable duties as requested by the Chief Pharmacist, Principal Pharmacist in Clinical Services or Head ofPharmaceutical Services.
