                                                    IIUT GP Liaison and Team Clinician with NHS South Central and West Commissioning Support Unit in Reading / Oxford / Manchester / Leicester

                                                     This is a key post in the specialist IIU team providing a wide range of administrative and clinical advice and support, to primary care colleagues, across all service areas. The overall purpose of the post is to work in partnership with primary care colleagues to reduce significant variation in uptake between different geographical areas and different population cohorts with a particular focus on reducing health inequalities and in improving and sustaining 0-5 childhood immunisation uptake rates.   We are looking for nurses/midwives to join our exciting and expanding team on a full time (job share considered) working as GP Liaison & Team clinicians delivering the IIU intensive support package and project work.   The successful candidate will be responsible for working across a range of different service areas, identifying children who are missing all or part of their childhood immunisations and supporting general practice colleagues to work with parents/families to ensure any children missing immunisations are given the opportunity to be vaccinated. The role will involve working closely and in collaboration with multidisciplinary GP practice staff, as well as supporting facilitating change to achieve and then sustain immunisation uptake rates.   The post may require infrequent travel within the geography covered by SCW.   About us  Working collaboratively with SCWs Child Health Information Services (CHIS) and data informatics team, the IIU team clinician will work closely and collaboratively with the Senior IIU clinician and all members of the IIU clinical team to assist with clinical support across all areas and also identifying within agreed service models which GP practices/areas would benefit most from focused intervention and support. The IIU Team clinician will work alongside all IIU Team colleagues to ensure GP Liaison and team processes are effective, productive, and collaborative in order to meet the objective of the IIU Team- to reduce variation in vaccinations and improve immunisation uptake rates across all service areas.
