The Buckinghamshire Healthcare NHS Trust is seeking a Band 7 Physiotherapist, or a Band 6 with community experience, for the Rapid Response and Intermediate Care team on a six-month fixed-term contract, covering maternity leave. The primary focus of the role is to manage a community physiotherapy caseload, providing expert interventions and rehabilitation to support patients in their homes, prevent unnecessary hospitalisations, and facilitate early hospital discharges.

In this dynamic position, the practitioner will collaborate within a multi-disciplinary community team that includes various healthcare professionals. Key responsibilities include conducting triage and comprehensive assessments, planning care programs to promote independence, case managing patients, and engaging in the development of clinical pathways.
