The Care Coordinator for Havering Liberty PCN at Havering Health Ltd in Romford will assist in coordinating patient care and support activities within the Primary Care Network (PCN). This role involves supporting the PCN Manager and Clinical Director in delivering various administrative tasks, managing patient appointments, and enhancing service accessibility. The Care Coordinator will work with primary care professionals and multidisciplinary teams to identify and manage patient caseloads, develop personalized care plans, and facilitate access to health resources and support services.

Additionally, the role encompasses maintaining relationships with stakeholders, promoting quality improvement, and adhering to relevant health policies and regulations. The Care Coordinator will be based at St Georges Wellness Hub and will have the flexibility to visit member practices as required, engaging in educational development and safeguarding measures to support vulnerable individuals in the community effectively.
