The role of Clinical Team Leader for the Care Home Pathway at Surrey and Borders Partnership NHS Foundation Trust in Epsom involves providing specialized support primarily to individuals with dementia exhibiting challenging behaviors, as well as those with moderate to severe mental health issues. The position requires an experienced Band 7 Community Psychiatric Nurse to work collaboratively within a multidisciplinary team, offering expert advice and interventions, maintaining effective communication with primary care and other agencies. You will manage a defined, complex caseload, ensuring the delivery of holistic, evidence-based care that promotes the wellbeing of residents in care homes.

This role also entails facilitating clinical sessions, conducting assessments, and implementing treatment plans based on positive behavior support principles. You'll participate in formal reviews and meetings, including tribunals, while utilizing your specialized mental health skills to address challenging behaviors and meet the needs of older adults in care settings.
