Anticoagulation Service: The base location is the Beckenham Clinic /Kelsey Park Farmhouse, but candidate will provide domiciliary service across the Lewisham and Bromley boroughs.

The role of the Anticoagulation HCA is to work within the Anticoagulation Service carrying out Domiciliary visits to patients who are currently on warfarin.

Working to a Standard Operating Procedure (SOP), the Anticoagulation HCA will carry out capillary INR blood testing (finger prick blood testing) on housebound patients in their own homes and liaise closely with a Clinical Pharmacist to ensure the patients have the correct doses of treatment recorded in their treatment cards.

This is a part time role (ideally Monday, Tuesday ,Wednesday) predominantly in the Lewisham area, but candidate may also be required to cover Bromley area.

Candidate will have access to a pool car. There is also a possibility for occasional overtime and working across other services.

Assist in the organisation of domiciliary rounds to ensure smooth and efficient operations.

Plan the domiciliary routes and vist housebound patients at their own homes.

Adhere to the loan worker policy when out in the community ensuring their own and patient safety.

Where appropriate, set up clinic room for INR clinics.

Support the clinician during procedures following sterile/non-sterile area protocol.

Conduct a blood test using a finger prick technique and blood analysis using a Coaguchek machine.

Ensure safe handling of patient samples and sharps in line with BGPA policy Assist with the ordering of supplies to keep the clinics well stocked Provide a high quality and skilled level care to patients.

Act as a liaison between the house bound patients and a Clinical Pharmacist.

If necessary, call for additional clinical assistance action if patients are found or become unwell during a home visit.

Carry out a duty of safeguarding during patients home visits.
