Riverport Medical Practice in St Ives is seeking an experienced Diabetic Specialist Nurse to work 4-6 sessions per week. The ideal candidate should have completed or be pursuing a postgraduate diploma in a nationally recognized diabetes education program. Responsibilities include managing diabetes care, conducting health education, reviewing and adjusting medication, and providing expert clinical advice to patients and colleagues. The role requires the ability to operate within agreed practice limits and to collaborate in a multi-disciplinary team to ensure safe, high-quality healthcare.

The Diabetic Specialist Nurse will coordinate diabetic healthcare delivery, conduct medication reviews in line with evidence-based practices, and educate patients on managing diabetes and pre-diabetes. Key tasks involve monitoring blood test results, filing clinical correspondence, participating in care home ward rounds, and maintaining accurate patient records. The successful candidate will also promote a learning environment, engage in practice-based meetings, and contribute to achieving clinical targets such as QOF.
