Job description
We are looking to appoint an experienced Diabetic Specialist Nurse for 4-6 sessions per week.
The ideal candidate will have completed (or be undertaking) a nationally recognised diabetes education programme at postgraduate diploma level.
The role of Diabetes Specialist Nurse includes management of diabetes, health education, reviewing blood test results, medication monitoring, and dose adjustments. The ideal candidate would be a prescriber, or working towards this qualification.
The post-holder will operate within their own sphere of competence, and agreed acceptable limits of practice, to provide expert professional clinical advice to patients, carers, and colleagues.

- Work as part of a multi-disciplinary team to deliver safe and high quality care.
- Make referrals as necessary to other members of the primary and secondary health care teams as appropriate.
- Co-ordinate the planning and delivery of diabetic healthcare, demonstrating good shared-decision making.
- Reviewing diabetic medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
- Deliver health education to patients to support management of diabetes and pre-diabetes, and refer to national education programmes where appropriate.
- Request, review, and file blood test results as part of diabetic monitoring.
- File incoming clinical correspondence to the Practice, and action subsequent tasks or requests as appropriate.
- Participate in the care home ward rounds where appropriate.
- Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
- Ensure clinical systems kept up to date, recording and/or amending accurate details.
- Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice.
- Contribute to local and national practice targets clinical remit e.g. QOF.
- Engage and contribute in Practice-based learning and meetings.
