Band 4: £26,530 to £29,114 a year pa pro rata  PERMANENT  GOOD SALARY 

This is an exciting time to join a busy and well-established multi-disciplinary health team, supporting the health care needs of the people living in Herefordshire.

We have a brand new opportunity to join our virtual ward team as an associate practitioner. Our aim is to deliver high quality care to improve health & wellbeing and to support our patients to remain living at home.

The post holder will work within clearly defined boundaries of their associate practitioner competencies and carry out specific delegated clinical tasks and responsibilities as defined by the advanced clinical practitioner.

The assistant practitioner will be assessed as proficient to work at a higher competency than expected of a band 3 health care worker, they will work in partnerships with team members, service uses, general practice, multidisciplinary teams and private/voluntary agencies.

The post holder will be expected to work autonomously in a variety of setting including the patient's own home. Being able to work alone and travel in a rural area is essential.
