The role of "Dementia Specialist Practitioner - Memory Assessment Services" at Lancashire & South Cumbria NHS Foundation Trust in Fleetwood involves providing specialist assessment, diagnosis, and treatment for individuals with suspected dementia. The successful candidate will work across two locations, Fleetwood and Lytham, and will focus on addressing changes in memory, cognitive function, and behavior among older adults. In this position, the practitioner will be integral to a dynamic, multi-disciplinary team that delivers high-quality assessments and interventions in both community and clinical settings. The role operates Monday to Friday, 9am to 5pm, and is part of a continual effort to enhance the support and well-being of individuals with dementia and their families.
