The Nottinghamshire Healthcare NHS Foundation Trust is seeking a dedicated Care Co-Ordinator for their Early Intervention in Psychosis (EIP) Service, focusing on individuals aged 14-65 experiencing their First Episode of Psychosis (FEP). The successful candidate, who should have a background in Mental Health Nursing, Occupational Therapy, or Social Work, will manage a caseload, conduct comprehensive assessments, and support the recovery journey of service users while working within a multidisciplinary team. Key responsibilities include collaborating on clinical decision-making, using standard outcome measures to track progress, and traveling across Nottinghamshire to assist service users and their families.
