The Senior Nurse Practitioner role at Wirral Community Health and Care NHS Foundation Trust involves joining the urgent community response (UCR) team to provide critical care for acutely unwell patients in their homes, including residential and nursing facilities. Candidates should be experienced paramedics or nurses with strong clinical skills and a background in community healthcare. This position aims to support patients in remaining at home, focusing on rapid assessments and personalized care plans, adhering to the National Guidance for 2-hour Urgent Community Response.

The successful candidate will collaborate with a multi-disciplinary team, including therapists and social care professionals, to assess and treat patients at risk of hospital admissions, addressing various urgent health situations such as falls, mobility issues, and acute changes in health status. An opportunity has arisen for an experienced Paramedic or Nurse to join our urgent community response (UCR) team, a part of the Community Integrated Response Team. This post is open to the most suitable candidate. Please refer to your professions job description and person specification to support you with this application.

This role will work across the UCR and Frailty Virtual Ward as required.

We are looking for experienced paramedic or nurse candidates with strong clinical skills, experience and previous community work, who are patient and team focused. The successful candidates will demonstrate that they are willing to support innovation, and are committed to service improvement in an extremely interesting, and challenging environment. Your aim is to support patients to remain at home and receive the treatment they needs, recognising risk and the need for onward referral as appropriate.

The services provide care for acutely unwell patients in their own home (inclusive of residential and nursing homes), aiming to avoid unplanned admissions to hospital supporting earlier discharges from hospital and in line with the National Guidance for 2 hr Urgent Community Response guidelines.

The multi-skilled team of therapists, nurses, social care and health care assistants provide urgent assessments, care and treatment for people living in Wirral. People are assessed within two hours and personalised support is in place within two days (usually 24 hours).

The team can also provide urgent assessments within two hours for patients who experience any of the following:

Fall (in or outside of their home or place of residence)
Reduced mobility
Confusion / delirium
Worsening of dementia
Urgent catheter care
Diabetes deterioration
Urgent social crisis
Palliative patient in crisis
Urgent equipment requests

About us
You will use established pathways and processes to provide a wide range of nursing, community paramedic, therapy and social care assessments to patients who may potentially be at risk of a hospital admission.

The successful candidate will also work with other clinicians including Physiotherapists, Occupational therapists and social workers, whom are all co-located within the team.

We welcome applicants from a range of backgrounds including emergency nurses, primary care and acute medicine as well as nurses from community backgrounds who have experience in working within Integrated Services.
