The Senior Nurse Practitioner role at Wirral Community Health and Care NHS Foundation Trust involves joining the urgent community response (UCR) team to provide critical care for acutely unwell patients in their homes, including residential and nursing facilities. Candidates should be experienced paramedics or nurses with strong clinical skills and a background in community healthcare. This position aims to support patients in remaining at home, focusing on rapid assessments and personalized care plans, adhering to the National Guidance for 2-hour Urgent Community Response.

The successful candidate will collaborate with a multi-disciplinary team, including therapists and social care professionals, to assess and treat patients at risk of hospital admissions, addressing various urgent health situations such as falls, mobility issues, and acute changes in health status.
