The post holder is expected to deliver a high level of skilled and planned nursing care to all patients and their families. Supported by the Practice Manager, Partners and other Practice Nurses you will work autonomously within your sphere of competency, whilst being a support and resource to less experienced members of staff. You will be able to assess patients, carry out relevant tests and arrange follow-up whilst offering advice and information; demonstrating critical thinking skills in clinical decision making whilst working effectively as part of a multi professional team within a fast pace environment, in line with the protocols and policies and clinical guidelines. Overall the post holder will work collaboratively with the general practice team to meet the needs of the patients, delivery of policies and procedures and providing nurse leadership as required.

Act as a positive role model to staff and patients.

Assess individuals health needs using an evidenced based framework.

Provision of holistic care

Administer treatment within protocols, policies and clinical guidelines.

Review medication for therapeutic effectiveness appropriate to the patients needs in accordance with evidence based practice and clinical guidelines.

To maintain competencies associated with the role ensuring you are working within your own limitations / scope of practice.

Administer medication within Patient Group Directions or Patient Specific Direction and to understand these methods of delivering medication.

Demonstrate advanced clinical skills in aspects of chronic disease management appropriate to your scope of practice.

Demonstrate advanced clinical skills in all aspects in the delivery of the immunisations and vaccinations service.

Referral / sign posting to appropriate health professionals / services.

Provide supervision and support of Healthcare Assistants at the practice.

Ensure clinical protocols are adhered to at all times.

Work at all times within the NMC Code of professional practice.

To participate and assist in the setting of standards of patient care based on sound research findings.

To complete on-line and face to face training as required, maintaining competence and professional development.

Demonstrate flexible and innovative ways of working to provide access to the patient population.

Maintain confidentiality and trust at all times.

About us

Undertake clinical reviews of patients presenting with a range of conditions which may include: Long Term Conditions, Diabetes, Asthma, COPD, CKD, Hypertension, Cerebrovascular disease, Dementia and Coronary Heart Disease in line with practice protocols.

Provide a comprehensive range of nursing services which may include:

v Venepuncture,

v Electrocardiograms,

v Ear examination and irrigation (if appropriate),

v Testing and recording patients temperature, pulse, blood pressure and urinalysis,

v Minor and complex wound management, including referral (where appropriate)

v NHS travel vaccinations including comprehensive advice to patients prior to travel,

v Family planning and womens health,

v Cervical cytology,

v NHS health screening,

v Promote and participate in the chlamydia screening programme,

v Smoking cessation advice,

Implement National and Local immunisation programmes for both adults and children under an individualised prescription or Patient Group Directive.

Provide nurse cover for treatment room, as required. This will include care for patients presenting with uncomplicated dressings.

Prevent inappropriate admission to hospital with patient education.

Identify vulnerable patients and set action plans for lifestyle changes.

Access and Support preparation, management and after care of patients having minor surgery on the premises.

Undertake opportunistic health screening of patients in line with practice policy and offer appropriate advice.

Support patients to adopt health promotion strategies that promote patients to live healthily and encourage self-care.

Provide an opportunity for patients to share emotional and psychological issues within a consultation.

Provide appropriate anaphylactic and emergency care according to practice guideline.

It is necessary to contribute fully in any local or national initiatives, procedures and targets for example, Quality Outcomes Framework (QOF) and enhanced services etc.

Recognise the signs of child abuse, family violence, vulnerable adults, substance abuse, addictive behaviours, suicidal tendencies and be aware of local, referral procedures.

Undertake annual training and demonstrate competency in anaphylaxis and resuscitation techniques.

Carry out home visits (where appropriate) for patients who are not able to attend the practice for routine nursing procedures where a visit from a member of the community nursing team is not appropriate.

Participate in the administrative and professional responsibilities of the Practice.

Ensure accurate and contemporaneous notes of all consultations and treatments are recorded in the patients notes on the clinical computer system as appropriate in accordance with NMC and local policies.

Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice.

Ensure collection and maintenance of statistical information required for regular and ad hoc reports.

Attend and contribute in Practice meetings as required.

Assist in the formulation of Practice values and behaviours, strategy and policy and develop appropriate protocols.

Ensure that efficient and appropriate stock control is in place.
