Livewell Southwest is seeking a motivated Social Worker to join their team in Plymouth, working 37.5 hours per week in an integrated health and social care environment. The role involves supporting health-funded individuals with diverse needs, including those related to learning disabilities, mental health, and long-term conditions. The successful candidate will engage in strengths-based practice, conduct complex assessments, manage care plans, and uphold statutory social care functions while promoting clients’ rights and well-being.
