The role of "Clinical Team Lead - Patient Flow" at Cambridgeshire and Peterborough NHS Foundation Trust involves managing and optimizing patient flow within three Intermediate Care services over a 12-month period. The post holder will work closely with the Operations Hub and the Intermediate Care Services, focusing on identifying and addressing internal constraints affecting patient flow, such as long length of stay and discharge delays. Responsibilities include establishing effective multi-disciplinary collaboration, overseeing discharge processes, conducting clinical audits, and ensuring compliance with quality standards.

Additionally, the Clinical Team Lead will be responsible for reinvigorating the "red to green" daily process, sharing best practices among services, and maintaining accurate patient records. The role requires proactive engagement with the Intermediate Care Services to enhance the internal patient flow journey while addressing any concerns or constraints that may arise.
