The Optimising Care Facilitator role at City Health Care Partnership CIC in Willerby focuses on improving health outcomes for individuals living with diabetes. The successful candidate will provide tailored education and support, applying behaviour change principles to empower patients in managing their condition more effectively. Responsibilities include delivering high-quality diabetes education, facilitating group and individual sessions on various related topics, and utilizing motivational interviewing and other therapeutic techniques to promote sustainable lifestyle changes.

Candidates should possess expertise in behaviour change management, excellent communication skills to build rapport with patients, and a commitment to staying informed on the latest developments in diabetes care.
