The Clinical Pharmacist role at Affinity Care in Shipley involves supporting a team of pharmacists and technicians within a general practice setting. The post holder will primarily focus on prescription management, medication reviews, and fostering relationships with stakeholders, with particular attention to patients with chronic diseases and complex polypharmacy. As a prescriber, the pharmacist will manage acute and repeat prescriptions, collaborate with various healthcare providers to enhance medication communication, and mentor less experienced colleagues.
