Medway Community Healthcare is seeking an experienced and motivated Community Palliative Care Clinical Nurse Specialist to join their dedicated Palliative & End of Life Care Team in Rochester. The role involves implementing evidence-based palliative care, supporting education and research initiatives, and working collaboratively with community teams to ensure high-quality patient and carer experiences. Candidates should possess strong communication skills, attention to detail, and the ability to work autonomously while coordinating care and providing specialist clinical advice.
