CASELOAD DEMENTIA MENTAL HEALTH MULTI-DISCIPLINARY NHS SOCIAL WORK
The Norfolk & Suffolk Foundation NHS Trust is seeking a Senior Community Mental Health Nurse/Practitioner to join their supportive Older People's Community Mental Health team based at Newmarket Hospital. This role involves delivering community-based mental health services to older adults with dementia and other functional mental health diagnoses in the Bury North area. The practitioner will work closely within a multi-disciplinary team, which includes various healthcare professionals, to provide high-quality care and foster innovative service development. The position requires strong assessment skills, case management experience, and the ability to work autonomously while being an integral part of the team. Candidates should have a passion for improving the lives of service users and possess the flexibility to collaborate effectively in a dynamic environment.

The role encompasses responsibilities such as managing a caseload under Care Programme Approach (CPA) and Non-Care Programme Approach (NCPA), utilizing recovery care planning techniques, and maintaining effective communication with service users and other professionals. The practitioner will also participate in multidisciplinary team (MDT) meetings, manage risk assessments, and administer depot medication.
