The Hospice of St Francis in Berkhamsted is seeking a compassionate and experienced Social Worker to join their Family Support Team. The successful candidate will work full-time (37.5 hours per week, with part-time options considered) to assist a qualified social worker and the Multi-Disciplinary Team (MDT) in providing high-quality support within the In-Patient Unit, community settings, and the Spring Centre. Responsibilities include collaborating with team members, advocating for person-centered care, attending care planning meetings, conducting assessments, and offering guidance to patients and their families. The role requires a thorough understanding of relevant legislation, safeguarding processes, and the ability to maintain accurate records and professional relationships.

Working alongside an experienced Social Work Assistant, the Social Worker will play a crucial role in addressing the emotional, practical, social, and financial needs of patients and their families during challenging times. The position demands a commitment to best practices in hospice policy, service development, and strategic objectives, while also promoting the organisation's values. Candidates must have the right to work in the UK and will need to undergo an enhanced DBS check.
