Job description
We are looking for a dedicated and compassionate individual with experience in health and social care to join the Family Support Team.
You will assist a qualified social worker and the Multi-Disciplinary Team (MDT) to deliver a high-quality service within the In-Patient Unit, the community, and our Out-Patient Centre (Spring Centre).
This is a full-time role (37.5 hours per week) however, we will consider part-time (30 hours per week) for the right candidate).
We do not offer sponsorship of any kind, the selected candidate must be authorized to live and work in the UK without restrictions.
- Collaborate with the Social Work Assistant and MDT in various settings.
- Advocate for person-centered care and support planning.
- Attend and facilitate care planning and MDT meetings.
- Ensure compliance with safeguarding and mental capacity processes.
- Conduct assessments and address various aspects of individuals circumstances.
- Provide advice, education, and support to patients and their support networks.
- Liaise with hospice, local authorities, and community services.
- Act as a resource for social care information.
- Use Microsoft Office and electronic patient records.
- Apply knowledge of relevant legislation in patient care.
- Maintain current knowledge of social care policy and legislation.
- Provide psychosocial and practical advice to wellbeing groups.
- Ensure tasks align with best practices and hospice policies.
- Support service development and strategic objectives.
- Maintain professional relationships and accurate records.
- Participate in supervision, team meetings, and appraisals.
- Handle sensitive information in compliance with regulations.
- Stay updated on policy, legislation, and governance changes.
- Promote charity values and act as a role model.

You will be joining an experienced and innovative Multi-Disciplinary Team and will help to identify and meet the emotional, practical, social and financial needs of those we support both patients and their families. You will be working alongside an experienced Social Work Assistant.The post will involve assessments and making referrals to external agencies and providers to ensure we provide the right support, at the right time and considering the appropriate legislations. Please note this post is full time working on-site at the Hospice.
Please note the successful candidate will be required to undertake an enhanced level DBS check.
