The Homefirst Assessor role at Cornwall Partnership NHS Foundation Trust in Hayle is aimed at physiotherapists, occupational therapists, or nurses with community experience, who want to enhance their assessment, care planning, and reablement skills within an integrated, multidisciplinary team. The position involves conducting assessments, planning, and implementing care for adult patients in their homes or care settings, while collaborating with various health and social care services. Candidates should possess strong organisational skills and a background in older person's services, and they will receive structured training and supervision opportunities.

This part-time position involves working 28 hours per week on a 7-day rota, which includes weekends and some bank holidays, requiring flexibility in scheduling. The role covers the Penwith area, including Hayle, Penzance, and St Ives, with working hours from 8 am to 6 pm. Applicants must be adept at triaging, assessing, and creating evidence-based treatment plans to support the care needs of the local population.
