The Hertfordshire Partnership University NHS Foundation Trust is seeking a dedicated Band 6 Memory Nurse to join their Countywide EMDASS Team in Stevenage, serving North Hertfordshire. The role involves conducting assessments for individuals with suspected dementia, providing diagnoses, and delivering post-diagnostic support in collaboration with a multi-disciplinary team that includes clinical specialists and partner agencies. The successful candidate must have strong organisational and communication skills, clinical expertise, and the ability to work independently. Responsibilities include completing risk assessments, creating care plans, and supervising junior staff, along with contributing to service transformations and maintaining a high standard of documentation and record-keeping.

Candidates should demonstrate a solid understanding of dementia care, including legal and ethical considerations, and are expected to engage in ongoing professional development. The Memory Nurse will also play a crucial role in fostering a learning environment, participating in clinical audits, and representing the team in various meetings.
