The Clinical Nurse Specialist will engage in clinical governance, develop care guidelines, and utilize audit processes for service evaluation and enhancement. Overall, this position requires a dynamic and experienced nurse capable of advancing specialist cardiac care while improving patient experiences within the ACS/RACPC pathway.
A permanent position has emerged for a Cardiac Specialist Nurse in the Acute Coronary Syndrome (ACS)/Rapid Access Chest Pain Clinic (RACPC) service, that is continuously seeking to improve patient pathways and experiences.
The position holder is a highly experienced nurse who can provide a comprehensive ACS/RACPC service and advance to an expert level of enhanced practice. The role entails conducting a thorough chest pain assessment, applying specialist knowledge and skills, making decisions through effective interdisciplinary care, and leveraging clinical and managerial expertise to facilitate treatment of a defined caseload of patients' health and care needs.
The position holder must have a strong foundation in providing direct clinical care, be able to perform advance physical assessments, possess IRMER, and Non-medical prescribing either has them or working towards achieving. Aspects of management, research, leadership, and education from all four professional practice pillars within the speciality will be included in the role. In addition, it will entail actively taking part in clinical governance, advancing lifetime learning, and offering assistance and direction to the interdisciplinary team.
o To post holder will work collaboratively across the organisational boundaries with the health care teams in the secondary and tertiary care to established system of care that supports referral and management of patients with ACS.
o To facilitate the prompt identification, timely treatment and appropriate referral of patients who present with ST/ non-ST elevation ACS in line with the NICE guidance..
o To demonstrate expertise in utilising advance assessment skills, implementation, and request agreed relevant diagnostic procedures/clinical investigations on the ACS/RACPC pathway.
o To work within the multidisciplinary team to identify the gaps in provision, best practice and the resources required for effective management of patients in the ACS/RACPC service in accordance with NICE guidance.
o To develop guidelines/ pathways to enhance optimal management and standardise care for patients in your caseload.
o To use audit as a tool to evaluate the service and improve practice
o To participate in the delivery of the ACS/ RACPC service in conjunction with the MDT.
o To provide health education and specialist advice on risk factor modification for patients attending the RACPC, referring to other specialist services as required.

- To take responsibility for the assessment, implementation and evaluation of care for patients on the RACPC pathway
- To establish a and work autonomously, with a defined patient caseload, as appropriate.
- Develop proficiency as an expert practitioner in RACPC management to provide an advanced level of knowledge and skills
- Using advanced cardiac skills to carry out patient assessment including history taking, physical examination and the requesting of agreed relevant diagnostic procedures/clinical investigations for RACPC patients.

- To work to ensure that all patients in your caseload have equitable access to investigations, treatment and follow-up care.
- To work within the multidisciplinary team to deliver RACPC service that will support and advise patients and their family
- To act as the patients advocate, when relevant, and be able to educate on all the treatment options to enable a patient to make an informed choice
- To work to develop best practice in line with NICE guidance for RACPC
- To work to identify both the gaps in provision and the resources required for effective management of patients in the RACPC service in accordance with NICE guidance
- To develop guidelines/ pathways to enhance optimal management and standardise care for patients in your caseload
- To use audit as a tool to evaluate the service and improve practice
- To ensure patients and their families are given appropriate oral and written information regarding their diagnoses and planned care
- To refer appropriate patients onto the cardiac rehabilitation nursing team
- To participate in the delivery of the RACPC service in conjunction with the MDT
- To utilise advanced cardiac nursing skills to assess and triage referrals made to the service and prioritise these according to clinical need
- To liaise with medical staff regarding further clinical investigations and treatment plans and organise as required
- To ensure that patients understand their diagnosis and treatment plan, starting the process of informed consent for further investigations and treatment for patients identified as having cardiac symptoms
- To provide health education and specialist advice on risk factor modification for patients attending the RACPC, referring to other specialist services as required
Work force
- To manage and supervise junior staff as required.
- To attend meetings relevant to the role and represent views of others in formal surroundings
- To contribute both formally and informally to the education, training and development of clinical staff across the integrated pathway of care.
- To contribute to the implementation of NICE guidance requirements for these patients to ensure the continuous education of staff in appropriate clinical areas in relation to their understanding and utilisation of evidence-based practice
Financial
- To use resources effectively
- To demonstrate efficient and effective management of the demands of the post e.g. patient caseload, service development, audit and teaching.
- Advise Directorate management team of any recommendations which may impact on the resources needs for the care and treatment of patients in your caseload
Performance
- To identify patients with a primary diagnosis of ACS
- To provide a comprehensive nursing service for patients with coronary artery disease and be a point of contact for RACPC advice to all professionals across the pathway
- To manage patients effectively attending the RACPC service
- To work in close liaison with the multi-disciplinary team across the pathway in order to meet the needs of patients and their families, and to aim for seamless care.
- To use clinical judgement and skills in decision making to manage this caseload of patients.
- To use skills of history taking and physical assessment to direct appropriate intervention.
- To work in collaboration with Consultant Cardiologist to optimise the medical management for the RACPC service
- To vigorously promote the patients participation in decision-making, enabling informed choice of treatment.
- To contribute to any audits requested by the Trust, Commissioners or Nationally
- To contribute to any relevant national / international research projects related to your patients caseload currently running within the Trust as needed
General
- To identify areas of research/audit with the aim to improve evidence-based care.
- To contribute to the development of local and Trust policies in relation to the implementation of the NICE guidance.
- To be contactable via a bleep system
- To access clinical supervision within the Trust
- To keep up-to-date with new innovations/research within the field.
- To undertake research projects where appropriate, with other members of the cardiac team.
- To develop and maintain own clinical skills, academic knowledge and expertise as appropriate.
- To act in accordance with the NMC code of professional practice for nurses/midwives.
- To review and maintain standards of documentation in accordance with NMC and Trust standards for records and record keeping.
- To comply with all policies and procedures, eg Health and Safety at Work Act and the Data Protection Act.
- To exercise sensitivity and diplomacy regarding role boundaries with other healthcare professionals across the Trust
