We have an exciting opportunity for a Health Care Assistant (Band 2) to join the Intermediate Care Team based at East Riding Community Bedded Unit within East Riding Community Hospital. The Intermediate Care service operates across Hull and East Riding providing care for patients requiring rehabilitation either in a community bedded unit or own home.
Working in an active therapy and rehabilitation unit will give you the opportunity to look after patient requiring level 4 rehabilitation, stroke rehabilitation and also patients receiving end of life care.
The unit is covered 24 hours a day, 7 days a week. We provide a range of shift such early, late, long day and night shifts.
The post holder will work as a band 2 Health Care Assistant in the East Riding community hospital Community Bed service.
The post holder will be responsible for providing personal care and supporting the rehabilitation of people with complex health needs to enable independence and a safe discharge into the community.
