City Health Care Partnership CIC is seeking a Health Care Assistant (Band 2) to join their Intermediate Care Team at the East Riding Community Bedded Unit within East Riding Community Hospital. This role involves providing personal care and support for patients requiring rehabilitation for various complex health conditions, including stroke recovery and end of life care. The service operates 24/7, and the position offers a variety of shifts including early, late, long day, and night shifts.

As a Health Care Assistant, the successful candidate will play a crucial role in helping patients achieve independence and ensuring a safe discharge back into the community. We have an exciting opportunity for a Health Care Assistant (Band 2) to join the Intermediate Care Team based at East Riding Community Bedded Unit within East Riding Community Hospital. The Intermediate Care service operates across Hull and East Riding providing care for patients requiring rehabilitation either in a community bedded unit or own home.

Working in an active therapy and rehabilitation unit will give you the opportunity to look after patient requiring level 4 rehabilitation, stroke rehabilitation and also patients receiving end of life care.

The unit is covered 24 hours a day, 7 days a week. We provide a range of shift such early, late, long day and night shifts.

The post holder will work as a band 2 Health Care Assistant in the East Riding community hospital Community Bed service.

The post holder will be responsible for providing personal care and supporting the rehabilitation of people with complex health needs to enable independence and a safe discharge into the community.
