The Buckinghamshire Healthcare NHS Trust is seeking a Senior Band 6 Occupational Therapist to join the newly established Frailty Team at Stoke Mandeville Hospital, specifically within the Same Day Emergency Care (SDEC) setting. This role will involve working collaboratively within a multidisciplinary team that includes physiotherapists, nurses, and clinical support workers to provide comprehensive assessments and develop swift rehabilitation and discharge plans for patients with complex needs. The successful candidate will undertake a variety of assessments and utilize clinical reasoning and evidence-based practice to formulate treatment plans, ensuring a focus on both clinical and social needs to enhance patient outcomes.

The position offers a supportive and dynamic work environment, with opportunities for professional development through integrated supervision, training, and the use of outcome measures.
