Patient centred care 6 Partners 3 Salaried GPs and 1 ANP 13,500 Patients Own list system Job Description 1. To see patients in accordance with the normal pattern of working at the Glebe Surgery, which operates a list based system, and to undertake home visits and telephone consultations, as well as an equitable share of duty doctor days and also to provide an equitable share of extended access appointments when required. 2. To record consultations and patient contacts in a timely and accurate manner on the practice computer system. 3. To regularly monitor results received electronically and act upon them as appropriate. 4. To read incoming mail relating to patients and act upon this in appropriate and timely manner. 5. To initiate referral letters in a timely and appropriate manner. 6. To deal with patient notes, messages and administrative matters as required. 7. To undertake reports, Cremation forms and other administrative duties, as required. 8. To process prescriptions daily, as required. 9. To liaise with members of the Practice team to ensure patient care and satisfactory administration of the Practice. 10. To liaise with the Practice Manager in relation to ongoing administrative matters, holidays, absences and other matters related to the smooth running and organisation of the Practice.
