The role of a Senior Practitioner at Essex Partnership University NHS Foundation Trust in Colchester involves working as a Band 6 Senior Community Nurse within the Crisis Response Team, delivering high-quality, evidence-based holistic assessments to patients experiencing mental health crises. Responsibilities include assessing, planning, implementing, and evaluating tailored care packages in accordance with professional guidelines and Trust policies. The position requires collaboration with the Hospital Liaison and Home First teams to provide a comprehensive urgent care pathway and involves managing the 111 Option 2 service calls.

The practitioner will also take on day-to-day coordination of the team's operations and will be responsible for triaging referrals, documentation, risk assessments, and providing education and support to service users and caregivers. Additionally, the role includes mentoring newly qualified nurses and participating in audit and research activities to enhance service provision, with a commitment to delivering continuous care as part of a dedicated 24/7 service. Driving is essential for community assessments, and the team has access to a lease car for this purpose.
