The role of Community Cardiac Nurse - Cardiac Rehabilitation and Anticoagulation at Medway Community Healthcare in Gillingham focuses on providing specialist care to patients with long-term cardiac conditions. The successful candidate will engage in evidence-based cardiac rehabilitation across various phases and contribute to exercise and secondary prevention initiatives. Responsibilities also include home visits to assess patients' progress, optimize medication, and support lifestyle changes, as well as participating in communication activities and audits related to the cardiac rehabilitation services.

Moreover, the nurse will be part of the anticoagulation team, responsible for managing patients on warfarin or other oral anticoagulants. This includes assessing patients, conducting near-patient blood sampling for INR readings, and implementing care protocols.
