The role of Specialist Liaison & Diversion Practitioner at Kent & Medway NHS & Social Care Partnership Trust involves working within the Criminal Justice System across Kent and Medway to provide critical support for individuals with mental health needs, learning disabilities, and other vulnerabilities. The successful candidate, who should be an experienced Social Worker, Mental Health Nurse, or Occupational Therapist, will collaborate with a dynamic team and various professionals in health, justice, and social care to assess client needs, facilitate care pathways, and encourage engagement with necessary services. The position requires flexibility, a proactive approach to risk assessment, and the ability to work effectively in settings such as police custody suites, courts, and prisons.

Candidates must possess a full UK driving licence and have access to a vehicle for work purposes. While prior experience in liaison, crisis intervention, or forensic services is advantageous, the primary focus of the role is on point-in-time assessments without holding a fixed caseload. Practitioners will play a vital role in linking stakeholders and influencing service users' experiences in the criminal justice system, with the aim of reducing reoffending and fostering safer communities.
