We are a CQC Outstanding training practice, operating across 2 sites in Hyde and Denton, Tameside, Greater Manchester, close to all major motorway network links and within easy access of both the cosmopolitan Manchester city and the beautiful rolling hills and moors of the Pennines
We currently have 5 partners, 1 Prescribing Pharmacist, 1 ACP and 1 PA alongside a full complement of PCN staff. We pride ourselves on providing a high standard of GP led care.
- In accordance with the practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
- Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
- Assessing the health care needs of patients with undifferentiated and undiagnosed problems
- Screening patients for disease risk factors and early signs of illness
- In consultation with patients and in line with current practice disease management protocols, developing care plans for health
- Providing counselling and health education
- Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
- Recording clear and contemporaneous consultation notes to agreed standards
- Collecting data for audit purposes
- Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)
- In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.

- We are a highly performing GMS Practice and have a dynamic team who are forward thinking, pro-active and are working to find new and smarter ways of delivering health, social and self care.

- Large nursing team, comprising of a Nurse Practitioner, Advanced clinical practitioner and a Prescribing Pharmacist .We also have 7 HCAs/Phlebotomist working across both sites.

- Awarded the best Patient Participation Group 2in the UK awarded by the National Association of Patient Participation.
- We work within Hyde PCN and have a close relationship with the local practices, awarded Best Primary Care Team 2020 by the BMJ awards.
- Training practice for ST3, ST2 and medical student year 5.
- No weekend work or evenings (this may be subject to change depending on the GMS contract changes).
