Wantage Primary Care Network is seeking a Social Prescriber to join its newly established Integrated Neighbourhood Team (INT) in Wantage. The role focuses on improving health and wellbeing for patients with specific needs by facilitating access to non-clinical community services and resources. Social Prescribers will work closely with patients to create personalized care plans, build trust, and provide support as they navigate community-based activities, health services, and other resources. The position requires managing an individual caseload, collaborating with multidisciplinary teams, and promoting the benefits of social prescribing across the network.

Key responsibilities include taking referrals from various agencies, providing one-on-one support to individuals and their families, helping them identify and address social determinants that affect their health, and fostering relationships with local organisations to enhance community offerings. The Social Prescriber will also engage in community development efforts and data collection to measure the impact of services, while adhering to data protection regulations. This role is crucial for empowering patients to take control of their health through holistic, community-focused interventions.
