The Leeds Community Healthcare NHS Trust is seeking a part-time Weight Management Facilitator to join their Tier 3 Weight Management Service in Leeds. This permanent role, offering flexible hybrid working, involves supporting adults with severe obesity (BMI of 35 and above) through a comprehensive 12-month program led by a multi-disciplinary team. The facilitator will engage in multidisciplinary teamwork, conduct home visits for housebound patients, and run metric clinics to assess patient health metrics. Responsibilities also include facilitating patient feedback, offering information about the service, and contributing to the development of resources and training.

Candidates should possess experience as a healthcare assistant and a willingness to provide sensitive and professional care. The role emphasises strong communication skills and collaboration with the wider team to ensure the delivery of effective weight management services. Full training will be provided, and staff will be equipped with the necessary technology for remote work.
