Band 4: £26,530 to £29,114 a year per annum  PERMANENT  GOOD SALARY 

You will be responsible for assisting with the provision of mental health support and

assessment for adults who are experiencing a mental health crisis under the guidance of a senior member of the Team.

The role will encompass attending a service user's home with a senior clinician to help complete domiciliary mental health assessments. The Crisis Response Team also manages

calls to the 111 Option 2 service and you will be expected to receive calls made to this service under the supervision of qualified staff.

You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hours a day service for 365 days a year. Shifts are currently 7am to 19.30pm and 7pm to 07.30 am.

In delivering your duties you are expected to display behaviours that are aligned with the Trust values and as required in accordance with the NHS Constitution at all times.

Core Clinical Functions

Assist in the management and care of service users under supervision providing Telephone support to patients in crisis, taking referrals from other professionals and statutory bodies.

Produce accurate and timely documentation / records on all clients in line with

relevant codes and Trust policy

Assist senior staff in the ongoing assessment and observation of service users and

provide timely and appropriate feedback

Assist in the risk assessment of the individual and of the potential hazards in the SU environment, then documenting and reviewing risk assessments

Provide practical support to service users, carers

Attend Team meetings i.e. referral / review / business as appropriate

Report and document incidents as required

Undertake physical health care checks of service users, document and report results

Under supervision take part in assessing and reviewing care.

Help create and provide resource information to service users and families

Undertake crisis calls and documentation of these

Core Non-Clinical Functions

Take part in service development and review

Ensure the service meets the agreed performance / contractual targets and adheres to local and government initiatives

PROFESSIONAL DELIVERY FRAMEWORK

In addition to core clinical and non-clinical support the following are additional key activities identified as being necessary to the role in the delivery of your normal day to day activities.

Complete mandatory training in line with the Trust policy

Receive annual appraisal

Undertake training relevant to Band 4 AP competency framework

Take all allocated annual leave

Receive management supervision

Attend seminars / conferences and other CPD events external / internal

Attend regular key Trust-wide meetings / forums i.e. Professional nurse advisory committee, nurse forum, Link nurse forum (Infection control)

Attend additional in-house developmental training, or support delivery of a number of mandatory training e.g. STORM, TASI, Infection Control, Safeguarding

Be an Infection Control Link person (to support 1st level nurse)
